Every shift change is a gap. One nurse leaves, another arrives. What happened at 2 AM? Was the PRN given? Did she eat? Did the pain come back? The outgoing nurse remembers most of it. The incoming nurse hears a summary. The family gets a phone call that starts with "she had a good day."

In hospice and home care, the informal handoff — a text message, a voice note, a sticky note on the kitchen counter — is the only record that exists. It is not enough. When something goes wrong, that record is what protects the nurse. It is what the family points to. It is what matters.

Most independent care teams today use a shared Google Sheet. Some use a notebook. Some use nothing at all.

How It Works

Plug it in.

Handoff arrives ready. Plug it into any outlet in the home, connect it to the home WiFi using a simple wizard on your phone, and it's live. No router login. No IT department. No laptop required.

Every caregiver logs in from their phone.

The lead nurse creates the care team in two minutes. Each caregiver gets a role — admin, caregiver, or family. They open a browser on their phone (no app download required) and log vitals, medications, ESAS scores, and diary notes directly from the bedside.

The record builds itself.

Every entry is timestamped and attributed. Vitals, PRN logs, pain scores, skin checks, nutrition, elimination — every clinical detail lives on the device in the patient's home. Nothing is lost at shift change. The incoming nurse sees exactly what happened.

The family knows what's happening.

Every night at 10 PM, a plain-English summary goes out by email: what was documented, how symptoms trended, anything the nurse flagged for the family. Every Sunday, the full week's records export as a CSV. The family sees the real picture — not a summary of a summary.

For the care team

Handoff is built to the standard you already hold yourself to. It documents what a paper chart would, in the time it takes to wash your hands.

  • Care log with 7 clinical domains — vitals, pain, respiratory, neuro, nutrition, skin, and elimination. Every entry timestamped, every entry attributed to the caregiver who made it.
  • ESAS assessments — the full 9-symptom Edmonton Symptom Assessment System, scored 0–10. Logged with every assessment, trended over time.
  • Medication list + PRN logging — document administration with before and after pain scores. A complete record of every PRN given, when, and by whom.
  • Caregiver diary with voice dictation — for the details that don't fit in a dropdown. Speak a note hands-free. It transcribes and timestamps automatically.
  • Append-only records — caregivers add entries. Admins correct errors with a note. Nothing is silently deleted. The record is the record.
  • Audit trail — every action in the system is logged with who did it, what they did, and when. If the record is ever questioned, the answer is there.
  • Multi-user roles — admin (lead nurse), caregiver, and family (read-only). One device, one care team.

Privacy & Trust

Your patient's data stays in your patient's home.

Every care record — every vital, every diary note, every medication log — lives on the Handoff device. Not on a server in another state. Not in a cloud database owned by a company you've never heard of. In the home where the care happens.

This is not a limitation. It is a deliberate design decision. Home care patients are among the most vulnerable people in any community. Their health records should not be exported to third-party infrastructure unless they choose that.

When you buy Handoff, you are not agreeing to let anyone mine your patient's records. You are not accepting terms that change next quarter. The data leaves the device twice: once each night in the family's summary email, and once each Sunday in the CSV export — both to addresses the admin controls. That's it.

For nurses: this is what a proper documentation record looks like for a home care setting. It is more defensible than a Google Sheet shared over text message, and it does not require your patient to consent to a third-party cloud service.

Why We Built This

My mother had the best care team I've ever seen. They managed everything in a Google Sheet.

When my mother went into home hospice care, she had a small team of independent nurses who were, by any measure, exceptional. They cared. They communicated. They showed up.

They also coordinated everything through a shared Google spreadsheet sent over a group text thread.

When I watched them hand off at shift change — one nurse summarizing to the next in a kitchen doorway — I understood what could go wrong. Not because anyone was careless. Because there was no system. The record was whatever someone remembered to write down before they left.

That team deserved a real tool. My mother deserved a real record. Handoff is that tool.
