Handoff

Built for the people who are actually there.

Independent nurses, CNAs, and the families who love them. Not for hospital IT departments.

For Nurses

Document without disrupting care.

Handoff gives you a clinical-grade record in a patient's home. Every entry takes about 45 seconds. Everything is timestamped, attributed, and permanent. Your liability is documented. Your team is coordinated. Your patient's data never leaves the home.

You're an RN or CNA working a case outside your employed hours. You've been using shared spreadsheets and text-message updates. You know that system breaks down. Handoff gives your team one place. Vitals, meds, diary notes — all timestamped and attributed to you. When you hand off to the next nurse, she reads your notes before she touches the patient.

No app to install. Open the browser. Log in. The form auto-fills your name, today's date, and the current shift. Log vitals in under 90 seconds. Use voice dictation for diary notes. Speak while you wash your hands.

  • Works on any iPhone or Android
  • Voice dictation for narrative notes
  • Large touch targets — works with gloves

For Admins

You're accountable. Make the record accountable too.

You're the lead on the case. You set up the team, you manage the schedule, you answer to the family. Handoff gives you the admin tools you need.

The lead nurse creates the care team in two minutes. Each caregiver gets a role — admin, caregiver, or family. You add and remove caregivers, manage the medication list, review every log entry with full audit trail, void incorrect entries (they're preserved, not deleted), and export the full record as a CSV at any time.

You set up the device once. Ten minutes. Then you never think about it again. The daily automated report emails the family for you. Every Sunday, the full week's records export as a CSV.

  • Admin controls who can access the record
  • Append-only audit trail — nothing disappears
  • Daily automated report emails the family for you
For Families

Know how Mom is actually doing.

Every night at 10 PM, Handoff sends your family a plain-English summary of the day's care — what was documented, how she was feeling, anything the nurse flagged. Not a summary of a summary. The real picture.

Your mother is at home. There's a team of nurses rotating through. You trust them — but you're not there, and nobody thinks to call unless something is wrong. Handoff emails you every evening with a plain-language summary: how she did today, what was given, how she seemed, who was there and when.

You can also log into the app from your phone and see the latest entries in a simplified view. No clinical abbreviations. Just: here is what happened today. Read-only access. You see what the nurses see.

  • Daily email summary — no app required
  • Read-only access to the care record
  • See medication names and when they were given
Daily Summary — June 2
Today was a stable day. Pain remained low at 3/10. Vitals were normal. Appetite was fair — she had soup and crackers at lunch.
The nurse noted she seemed more alert in the morning and asked about her garden.
Medications Today
Scheduled meds given on time. No PRN medications needed.

The record exists. Whatever happens.

Whether Handoff is ordered by the nurse or the family, it serves the same purpose: a complete, timestamped, accountable record of every care action.
